What the work costs, measured properly
The word burnout does a disservice to what is actually happening. It suggests something ran out that should have lasted, and it locates the fault in the person. What the evidence describes is narrower and less personal: sustained high-consequence decision-making inside a system with insufficient slack produces measurable change in the functions that decision-making depends on.
Under that load, judgment shifts from deliberate consideration to pattern-matching. Pattern-matching is fast and usually correct, which is exactly why the shift is hard to notice from the inside. The presentations that do not fit the pattern are the ones it fails on.
The second cost is identity. When the system fails a patient, the clinician standing closest absorbs it as personal failure — and carries it into the next decision, and the one after.
